VPT Student Concession Transportation Card

Does anyone know how I will be able to get a VPT student concession transportation application form since the RMIT sites are down?

I've been informed that I will need to download the application form from RMIT since they'll already have my details and photo on it, but my full-year myki is about to expire and I was wondering if I would be able to get that form in any other way possible?

I've tried to log in to Myrmit -->my service to download the concession form but it's still not working.
